1. The new patient form mailed to you OR a copy of the on-line registration printed out (if you did not email it to us).
2. Your current Insurance Information Card
3. Any eyeglasses that you wear or have worn, including sunglasses.
4. If you are a contact lens wearer, please wear your contacts to the appointment.  Bring with you the packages or boxes that the contacts came in.
5. If you are a new patient, please have your prior records sent to us.  They can be faxed to 860-677-0517.
